Wolves Wellness Student Based Health Center

WolvesWellnessLogoHSO LogoHealthSource of Ohio is proud to partner with the West Clermont School District to provide primary care services at our School-based Health Center.  A School-Based Health Center (SBHC) is a health care delivery model that acknowledges and supports this strong link between health and academics.   HealthSource operates a SBHC in the nearby Western Brown School District and has over 40 years’ experience providing care to communities throughout Clermont County. 

The SBHC is designed to treat students and school staff. Located at the West Clermont Middle School, the Wolves Wellness School-Based Health Center clinician team will provide medical, vision, dental, and behavioral health services where and when it is most convenient to students, their parents, and staff during the school day.  The SBHC is staffed by medical professionals that deliver primary care services which include screenings, managing chronic care conditions, and treating short-term illness.  The SBHC also offers 24/7 on-call coverage.

The SBHC clinicians do not replace your child’s primary care provider.  They do, however, offer a point of access at the school to treat illness and manage chronic conditions.  Our clinicians work with a child’s primary care provider to ensure continuity of care is maintained.  Students are only treated by the center after the parent or guardian enrolls the child in the program.  If treated at the SBHC, the clinician sends home a written summary of the visit for parents to share with the child’s primary care provider.  We invite parents and community providers alike to tour the center, learn more about the SHBC, and how we can all partner to ensure students remain healthy and ready to learn.
