Senior Immunization Requirements

We hope you are enjoying your summer.  The beginning of the 2018-2019 school year is quickly approaching. We know planning for your child’s senior year is an exciting time. We wanted to remind you of an important detail as we get closer to the start of the school year.  All students entering the 12th grade are required by Ohio law to provide proof of the following vaccination BEFORE the start of the 2018-2019 school year.

Meningicoccal (MCV4) vaccination

If your student does not receive the Meningicoccal (MCV4) vaccination by the beginning of the 2018/2019 school year, he/she may be excluded from school beginning September 4th, 2018 in accordance with Ohio Revised Code 3313.671.

If you have already turned in an updated vaccination form there is no further action required on your end.  If your child's immunization records are not up to date, we encourage you to contact your primary care physician to schedule an appointment for the vaccinations as soon as possible. 

You may also contact the Wolves Wellness Center at West Clermont Middle School at 513-732-5082 to schedule an appointment.  You may use your current health insurance at the Wolves Wellness Center.  However, if you do not have health insurance the vaccinations will be provided by the Wolves Wellness Center at no cost to you. Vaccinations are also available at many local pharmacies.

Once your child has recieved the vaccination please provide us with an updated copy of their immunization record as soon as possible.  You can send the record with your student to school once the year begins or you can fax your child's record to 513-752-3460, Attn: Health Clinic.
